An inevitable part of geriatric care management is end-of-life care.

End-of-Life Care

Our clients have a variety of issues and circumstances to address but, in virtually all cases, the single most important issue is the dignity and self-respect of the client.

At the beginning of care management services, we begin regular family meetings that establish a safe environment where plans, decisions, conflicts and grief issues can be discussed honestly and openly. When end-of-life draws near, the goal of these meetings evolves from ensuring dignity and quality of live to helping to ensure continuity of family support.

Why Choose Us?

Communication

During this process, there can be a lot of questions. We want to relieve any fear and confusion by explaining issues, expectations, and communicating openly with families.

Mediation of Regular Family Meetings

In this difficult time, it is important that the family can candidly discuss plans, decisions, conflicts, and grief issues honestly and safely.

Palliative/Hospice Care

At Family Connect Care, we advocate for our clients so they can feel comfortable and dignified throughout the end-of-life process.

An inevitable part of geriatric care management is end-of-life care. Our clients have a variety of issues and circumstances to address, but in virtually all cases, the single most important issue is the quality of life of the client. Saying goodbye to a loved one is never easy. The only things that make it easier are knowing that their wishes are being listened to and that they are comfortable in this end-of-life process.

Through this process, professionals at Family Connect Care will negotiate care, explain processes, and communicate all critical information to families. We also know how important it is for families to be involved, especially at this time, which is why we facilitate meetings with the family to encourage sharing of thoughts and feelings. By making decisions as a team, we can mitigate any conflict, ultimately leading to united plans to best support the loved one.

The beautiful thing about making these decisions together is the realization that you have options. Let us help you find the options that work best for you, your family, and your loved one. In this emotional time, know that you are never alone. Family Connect Care is here to help.
